Crown & Bridge have developed our unique 'Product Passport' for each and every Order.
This confirms that prostheses manufactured by Crown & Bridge Ltd and permanently fitted in your patients' mouths conform to EU Regulation and carry the relevant CE registration.
'Product Passport' Information:
- Our MHRA registration number.
- Our prescribing dentist's (client) details.
- Date of prescription (ie when the product is to be fitted).
- Patient details.
- Description of order for Patient.
- CE numbers and trade names of the materials used in the product's manufacture.
- MHRA statutory declaration.
